Well hello!
It's been a moment. If we worked together in the last year of my practice, you would have received an email in 2022, or, we would have discussed in person the closure of my practice. If our services were ongoing, we would have discussed referral options. My status with the College of Counselling Therapists of New Brunswick (CCTNB) is now Inactive, although I believe my license number is still the same should it be needed (LCT20-006). This is because I practice in Alberta as a Registered Psychologist, and this title supersedes any other title I have held.
For adults, I am required to retain your file for eleven years from the last date of service. For those under the age of eighteen, I am required to retain your file until two years after you have turned eighteen, or for eleven years from the last date of service, whichever is longer. They are archived should you ever need them for referral, to communicate mental health history to another professional, in the event of a psychiatric emergency, etc. Some files are kept indefinitely, and this depends on certain criteria set out in the Limitations Act.

I will need a signed release of information, or, in the event that you are a family member of a deceased client, a signed release from yourself with proof that you are the holder of the estate of my past client.
It was an honest privilege to work with you, to be a therapist in Fredericton, and to have walked alongside you in your journey. This is perhaps the hardest part about being a therapist, I don't get to see what happens next. I'd like to hope that you went on to find another therapist, that the work is spectacular, and that you're receiving the care you need.
